Registration and other forms

In the fall, at the beginning of the school year on Rally Sunday (for 2007 that's September 9), please download and complete the church school registration forms, and bring them to church.

Please complete registration forms for each child attending church school (nursery through seventh grade). There are four forms:

Registration (one per family)
Nursery (only for those in the nursery)
Health (one per child) 
Field Trip (one per child in Grade 1 or above).

If families complete paperwork in advance we hope they can better enjoy the Rally Day celebrations! And we will have better information to assist church school teachers in their efforts to know your children.

All these forms are available as individual PDFs (portable document files) which you can download to your computer, print at your printer, fill in, and bring to the church.
